It refers to the role of a handsome man with his face painted white.
In front of the theatre buildings in the Edo period, a wooden board was posted with the names of the actors written upon it. The actors listed on the first board were called Kakidashi (written at the top), while the white-painted handsome characters were usually written on the second board (nimaime), which is why they came to be called this way.
